Keep in mind that during the end of November until end of December postal services are very busy with Black Friday and Holiday packages, shipping times can be longer than average.

What are the shipping costs?

USA
UPDATE: Shipping to the USA is back, available with Fedex (flate rate and fast shipping) & USPS delivery.
I am shipping with a DDP service from Fedex and Postnl (with USPS delivery)
DDP means Duty Delivery Paid, which means you won't have to pay the courrier to have your order delivered, everything is paid at checkout.
Tariffs / duties and handling fees are collected upfront in checkout.
